What happens after the referral?

PHILL is developing MCAP — Mentor-Coordinated Access to Prevention — to test a problem healthcare organizations routinely encounter: What happens after the referral, before the next appointment, and between major preventive-health milestones?

PHILL does not propose becoming a healthcare provider. We are interested in the infrastructure surrounding healthcare engagement. MCAP is designed to explore trusted community access, participant preparation, nonclinical Professional Mentorship, referral progression, administrative navigation, responsive HELP when barriers emerge, participant-centered information continuity, and longitudinal engagement between preventive-health milestones.

What we need from healthcare partners

We are seeking organizations willing to help us design the clinical interface before we implement it. Questions include:

  • What constitutes an appropriate preventive-health milestone?
  • How should existing patients participate?
  • How should people without a primary-care home access care?
  • What information is actually needed for a referral?
  • What can PHILL appropriately verify?
  • What information should never leave the clinical environment?
  • How should status-neutral HIV prevention and care fit within broader preventive health?
  • Where does coordination currently break down?
  • What would make MCAP useful rather than another administrative burden?

What PHILL is not proposing

  • We are not asking partners to surrender clinical decision-making.
  • We are not proposing to steer participants away from existing providers.
  • We are not asking for unrestricted medical-record access.
  • We do not want to create another referral system that increases provider workload.
